Client Intake
All new clients must complete this intake before their first consultation, whether your session is in person or virtual.
The intake takes about 25–30 minutes and only needs to be completed once. Your answers become your client profile: the starting point we use to personalize your formulations and track your progress over time. You'll only come back to update it when you have new lab results.
Before you begin, have these handy:
- Your current medications and supplements, with doses
- Any allergies or sensitivities
- Recent bloodwork, if you have it (optional, but highly recommended)
The form opens in a new tab. If you'd like to upload lab results, you'll be asked to sign in to a Google account.
